Registration Form
All form fields must be filled out for this to be a valid renewal form.
Each club may send up to 2 delegates to the ACoD, however only member club delegates may vote. Affiliate clubs may also send delegates. Affiliate club delegates may make motions as well as comment on motions, however those delegates do not have a vote. Observers also may register, they are "eyes and ears only". Observers may not make comments nor vote.
Delegates registered for the ACoD do not have to be the club's liaison. A club may choose to send anyone they wish to represent them.
A non-spouse member of the club’s board must email Ping Pirrung at asfa.ping@gmail.com stating that [delegate’s name] will be representing [Name of Club].
The delegate’s registration will NOT be valid until the payment and email from the club’s board is received
REGISTRATION fee is $45
The deadline for receipt of the registration the 14th of March and in the mail by the 6th of March.
